Sleeve Plus Proximal Jejunoileal bypass (Sleeve-PJB) Service with Dr. Yussra Yusoff

The Sleeve Gastrectomy with Proximal Jejunoileal Bypass (Sleeve PJB) is a combination bariatric surgery designed to provide significant weight loss and improvement in obesity-related health conditions. This procedure combines two effective techniques: the restrictive nature of a Sleeve Gastrectomy and the malabsorptive effects of the Proximal Jejunoileal Bypass.

The procedure starts with a Sleeve Gastrectomy, where about 80% of the stomach is removed, leaving a narrow, tube-shaped stomach. This limits the amount of food you can eat at one time and reduces hunger by lowering the levels of the hunger hormone ghrelin. Following this, the surgeon performs a Proximal Jejunoileal Bypass, which involves rerouting a portion of the small intestine to shorten the pathway food takes through the digestive system. This bypass reduces the absorption of calories and nutrients, enhancing weight loss.

Patients can expect to lose 60-75% of their excess weight within the first 12 to 18 months after surgery. The Sleeve PJB is also highly effective in improving or resolving metabolic conditions such as type 2 diabetes, hypertension, and high cholesterol. The combination of restrictive and malabsorptive mechanisms makes this surgery a powerful option for long-term weight loss and health improvement.

As with other bariatric surgeries, Sleeve PJB requires a lifelong commitment to dietary changes and nutritional supplementation. Due to the malabsorptive component, patients must take daily vitamin and mineral supplements, including multivitamins, calcium, vitamin D, iron, and vitamin B12, to prevent deficiencies. A specific post-operative diet must be followed, starting with liquids and gradually progressing to solid foods. Eating smaller, nutrient-dense meals is essential, and avoiding high-sugar foods can help prevent dumping syndrome, which can cause nausea, diarrhea, and other symptoms. This combined procedure offers a robust tool for achieving significant weight loss and improving health but requires dedication and careful management to maintain long-term success.
